Activity Log
Part of the documentation process that you will need to do for your program is to keep an Activity Log. This is simply a tally of the various tasks that you took part in, with details such as the date, a brief description of the nature of work done, and the activity supervisor's signature.
Make sure to have an Activity Log for each activity that you intend to include in your CAS program.
(Even though the program has moved on from counting the number hours, this log will come in handy in case the school or university you want to apply to requires more concrete evidence of the actual time devoted to community service, for example.)

Just download this FORM and fill in the needed information. Then make sure to have your respective supervisors sign off on the sheet each time you do non-trivial work for your activity. (It is expected that you will fill up multiple copies of this log sheet over the 2-year course of your CAS program. You are strongly encouraged to print copies of this form back-to-back to save on paper.)
*For activities that meet daily or very regularly (like varsity sports, for example) you can make arrangements with your supervisor to have them sign off on your logs on a weekly basis instead of daily. For clubs that meet once or twice a week, supervisors must sign the logs every meeting.
